I need to know if most medium sized medical practices own their own servers or are in the cloud.
We have our own. and other offices I support or use my custom applications/add-on's have their own as well.
We have our own servers at a data colocation center. I think renting rackspace is pretty ideal. (Don't have to deal with HVAC, backup generators, etc.)
All servers are in house utilizing VMware
We host everything in house, but I would love cloud based DR!
VMWare here for every server (GE or not) except for my Biscom server. I do replicate my backups off site at a Co-location and replicate that back to a remote site which I manage.
Word of caution if you do consider a cloud; you should have a SLA in place with your ISP, unless losing access to your resources isn't important. Our ISP is very reliable but past performance does not guarantee future reliability. We are in FL and it would only take a hurricane take out internet access for a couple weeks or so.

Were the same VMware for everything but the Fax Server but we do have backup hardware in a different location.
