We are in need of a way to document when a physician has concerns about child abuse (for example) in the chart so other physicians that might see the patient are informed. Of course, they do not want the information to go to the portal or print on a CVS. Care Alerts is an option but abused by our clinics. Anyone come up with a creative way to do this?

Not an answer for today, but my understanding is the CEMR SP11 (perhaps also with upcoming CPS), there will be the ability to limit OBS, Problems, etc... that would pass to a patient portal. It will be a global/universal setting - in other words, for ALL patients.
As an aside, GE mentioned recently that a future SP would have more capability to control this with more granularity.
As I thought about this a little more, in the interim, could you:
(1) Add the problem 'Physical Exam for Camp', ICD10-Z02.89 to the patient problem list. I think this code is also used for 'Physical Exam for Employee'. So, if you think that you can use this code for a different purpose, without conflict, then good.
(2) Train staff to know that this problem is a surrogate for actual code.
(3) You could probably build something of a warning in the Patient Banner if the Problem code is active, and could be creative about the wording. Similarly, if you do VFE to design your own Office Visit Encounter forms, you could put some indicator for Physicians.
(4) Lastly, even if someone sees this on a form, blame it on a 'quirk of the recent ICD-10 implementation', saying there must have been a strange conversion of a past Physical Exam.
