Over the past 6 months we have been having a issue with immunization custom list population in the immunization HTLM form. The issue will present when a end user is looking at the immunization list and instead of showing our defaults- Adult, Pediatric & Travel it is populating a Adult Custom or Pediatric Custom. When we look at the Immunization Custom List in the settings you will see those erroneous Adult Custom/Pediatric Custom. Once you remove those it will show the ones we have built and the form is updated resolving the issue. Just trying to figure out the logic in which it is occurring and how to prevent it. At this time we have ruled out KB updates and server reboots.
