How do I add in the value of (DATE and DECLINED) for the patient declined immunizations on the Immunization History Form.

We don't have a form titled "Immunization History". What version of the forms are you on? Are you using the HTML Immunization Management form?

Hi, It is under the histories section and is called Immunization History-CCC. When a vaccine is declined it only shows a date not declined. I am looking to have both values populate. Thanks

Jessica, I believe that Debra is referring to the Histories View in the Tree on the left hand side. Open the Histories View and then select "Immunization History View" from the drop down.
