The following immunization, recently entered on this patient's chart, has been inactivated by the CDC and may not have been recorded correctly by the state immunization registry:

CDC Notes - This CVX code allows reporting of a vaccination when formulation is unknown (for example, when recording a HPV vaccination when noted on a vaccination card)

Is your organization linked to a state immunization registry?
yes
Are you using the CPS Immunization Form? We opted not to use the CPS Immunization form and have a 3rd party manage our inbound and outbound immunization records to the State Registry. We do not receive any flags through the EMR, however we get similar error reporting that is commuicated on a report "outside" of the EMR. We will then manually research and correct any errors in the EMR.
I think these may come from the Qvera interface engine.
trichards would you mind sharing who your third party is?
David I believe you might be right , thats what my IT department belives also
Those flags seem to be coming from the Qvera interface.
