Hello,
We're looking to add a release of information eform on our patient portal. Does any one have any examples of what their facility is using? And have you run into any issues regarding electronic signature versus a handwritten signature?

In EZ Access patient portal, we just have a form that looks like our normal form. The patient can sign using their mouse, finger, etc. We have had no issues requesting medical records this way.
