Is anyone using the medication reconciliation menu item to attest for meaningful use? How did you insert the CARETRANSIN observation into your workflow to indicate that the EP was receiving a patient from another setting of care or provider?

I wanted to add a button to the Med-Allergy form. Need the dlg file.

We added a question to our HPI form that medical assistants answer before entering the chief complaint and initial HPI information:
"Since the patient's last office visit, has he/she been in the hospital or seen a specialist?"
If the answer is yes, we also provide boxes to enter the name of the facility or physician. That isn't required for MU, but we found it helpful.

Our medical assistants were already verifying medication lists during the visit (in the Update Meds window), but they weren't always checking the med review box. To make the workflow a little more efficient, we also added a button on the HPI form that takes the user right to the Update Meds window. We trained staff to make sure the meds reviewed box was checked for every visit - not just transitions.
We did about the same thing other than having it on a Histories form that the first page has the transition into care box and data displays for Meds, Problems, Allergies, PMH, PSH, FH, SH.
The nurse can review and check meds/allergies reviewed this visit and if need be, add new items via a update meds button.

We added a checkbox for "This patient's Pain Management care is being transitioned from another provider" on our Vitals form - figured the MA's were there already, and they were used to checking the meds/allergies checkboxes at every visit anyway.
