We are running CPS 12.5 and are about to head down the pathway of setting up an interface to our state registry (NJIIS), and thus, will need to start using the HTML form.

By the way, is it possible to get a different custom immunization list (than the factory list) to display as default within the actual form?

I can hopefully save you some time. I spent several hours with a GE Technician today troubleshooting an issue in that form, only to find that it is actually a product defect (which was somehow unknown up until today.)
The problem is this: when a patient has received multiple instances of a vaccination (such as a series, or a yearly administered vaccine like flu), the historical view that comes with the new form will only show ONE instance of the immunization, and that one instance isn't even necessarily the most recent. In other words, a patient who came in last year for a flu shot, and again this year for a flu shot might show only having received last year's shot.
Everything else we've tested seems to work just fine, but based on this pretty severe charting issue, I personally wouldn't recommend making the switch. I'll update this thread with an SR once one is generated.
As to your second question regarding a custom list, yes. This is absolutely possible. It's very similar to generating a custom list for medication administration.
