We have just upgraded from 9.5 to 9.8 SR3 as of Wednesday night/Thursday morning and are having some difficulty with end users and the removal of problem qualifiers. I was wondering how others have begun to tackle this problem, no pun intended. We anticipated some growing pains getting used to no longer have rule out, history of, question of etc but not to the entent we have experienced thus far. To compound the issue one of our practices uses some forms they had developed through a consultant that filters problems based on these qualifiers so, effectively, those forms are now broken. As a general discussion point, what are people doing to replace this functionality? It feels like GE has removed a very important piece of functionality while giving us no replacement.

When possible, change to the appropriate V code for past or family history.
It is often not possible to do that, and one workaround is to simply overwrite the text attached to the code instead of using the prepend. The problem list can now show at risk for or PHx of or whatever.
Perhaps not elegant, but it at least keeps the problem list informative.
(I assume with no data that the reason for the change was to make sure numerators and denominators only reflect current, active diagnoses. The reporting tail wagging the medical care dog.)
