Have you checked your templates for extra closing braces? Performed MEL traces on your forms to ensure they are error free?
These are the two most common causes, but there are others, such as multiple users in the update using the same form, or forms with linked code/variables/obsterms, - a big no-no, of course.
In my experience, when this issue reveals its head, the session for the user is corrupted and it is carried like a disease being transmitted from update to update. Sometimes, having the user completely exit the EMR is enough to cure what ails them, but other times, especially when it turns into a pandemic across your userbase, because user 2 opened user 1's corrupted update, the cure is more drastic by booting everyone off the system and rebooting ALL machines and servers. Very painful and guaranteed to make you the most talked about individual of the day! :/
If none of the above provide relief, I would recommend contacting athena support for a data/database check up. It might be the canary in the coal mine...
