Anyone keep old versions of forms in their system?
If they are not being used anymore because newer versions were added, does it make sense to leave them in there? Maybe this is an audit thing for old documentation that used these forms?
We keep all forms in subversion so we will always have a copy if needed to refer back( even the old non used forms) . We try to remove all forms from EMR that are no longer being used however.
For reference and audit purposes, we keep all of our revised, replaced, and removed forms, letters, and handouts, etc.
I second placing old forms in some sort of version control. It helps prevent confusion for the staff, and they're there if needed.
Maintaining versions outside of the EMR is the best practice for back up and auditing. Only active forms should be in your system. If you are concerned about 'tampering', don't be, at least at the forms level. Every component of the EMR form is stored in the database 'as is' when imported. If someone was to import another version of the same form, a new row is created in the database and the previous version is marked inactive. It is still in the system, just not visible to the user. Subsequently, your audit trail exists, even with the form removed from 'view' (the row is simply marked inactive).
