Does anyone have any suggestions on how best to utilize the new problem list search engine capability? We recently upgraded directly from 10.1.3 to 12.0.5 (bypassed CPS 11) so have never been introduced to what appears to be a new ICD 9 or maybe even ICD 10 structure scheme. Are there any training resources? It's very frustrating. Type in breast cancer, nothing. You have to type in neoplasm breast in order to find it. Type in Diabetes, and you get 8 folders that you have to drill down into several layers of folders. Does GE offer training on the design and why it's changed?
I can't speak for your practice. But my providers hated!!!! it until I change there default custom list to "Smart List". It really does seem to get better the more you use it. Pick one of your more patient providers to test with and see if they find it better.

I have found some of the diagnosis codes are incorrect in the ""smart list"". A provider searched for "family history of CAD female 1st degree relative<60" which was assigned to ICD9 V16.59, which is a code for family history of urinary organs. Is there a way to edit this "smart list"??? Thanks
Well, I'm not sure about the editing. The way I understand it and I surely don't completely. But I think it looks through all the custom list and reference list as they are chosen. It may be a bad entry in one of the custom list as well. I checked that code in the reference list and it only shows "Family history of malignant neoplasm of other urinary organs"
So I would think its getting it from a custom list.

Thanks for all the replies. It's good to know people are actually out there surveying the posts. I do not use the chug portal enough, but will in the future. Unfortunately, the smart list still does not work very well for us. While it is better than not using it at all, its still not great, especially for our specialists. There has to be some sort of insructionsl training available somewhere on how to best utilize what we are now stuck with. Anyway, I will keep this post open in hopes of hearing other comments. what we learned about the incorrect codes on the smart list is that if a provider has picked an incorrect code previously, the smart list will remember it. To correct this, that provider must pick the correct code 3 times from the smart list, then the incorrect code will not show up. Painful process, but it works.
1. Use Smart List
2. Use Custom List (particularly if specialist)
3. Use Search magnifying glass (w/ICD-10 -- has better vocabulary but pick only those with accompanying 9s). Combine with Smart List above. Rinse and repeat.
4. Use Search magnifying glass (w/ICD-9)
5. Replace any 1-4 step with Google >> bring code back to EMR's Search magnifying glass
6. Await future software update
These are all great suggestions and we'll continue to fight the good fight. We have been creating custom lists for our specialists which seems to be the best avenue for them. Our family practitioners are all using the smart list to some degree of success, and when not successful, they use an icd search engine to find the code then input the code into the system. time consuming no doubt. I guess until any further software updates are available, we'll continue to take this approach. But again, I guess there isn't any official training out there that would better equip us with the knowledge to better use the existing search engine. It's just unfortunate that GE pushed out this new search capability without any information on how to use it!
The smartlist will get better the more its used. At least that is what we have experienced.

This has been a huge issue for us as well. One of our midwives brought me a list of 50-60 diagnoses that she couldn't find using the advanced search. Some of them were extremely simple and common. Once I discovered the smart list, it got a little better, but only a little. I'm not sure that instructions or documentation would really help all that much. It is just a poorly designed tool.
I think jamesedward (3 posts back) has the best approach. I got a list from our certified coders of all the icd codes we ever use in our specialty (obgyn) and created a custom list with our own descriptions. It is a very large list and took a long time for me to create, but I think it will help immensely.
Thank you jamesedward! Excellent advice. When you are stuck in the moment of trying to find a diagnosis you know you've entered so easily before...it's easy to forget there's always Google. If anyone has more explanation about how the smart lsit gets smarter I'd be interested in hearing it. Thanks.

Why would GE use MSDOS search engine technology? When will they catch up with modern search engine technology?
Our ten surgeons find it unacceptable and bad medicine. I have been tasked to find a solution if GE can't.
