We are starting to work on a policy for Opioid use/pain management contracts, etc. Does anyone have a pain contract, template or tools they are using to screen patients, or feedback they are willing to share?

This was the first VFE form I ever created so please excuse the computer code 🙂
It's based on the CDC checklist and has a link for providers to check the PMP w/o opening a new window. We probably need to update it based on the CMS requirements for prescribing, but this is our current state. I've also attached the handout that prints for the contact with our name removed.
Hope it's a helpful starting point!
