I am looking for input from those EMR/CPS clients that are tracking Pediatric Genetic patients. How are you keeping track of the patient's parent information? Are you storing them in just the clinical note or are you placing these items in OBS terms?
EXAMPLES:
Patient's mother's past pregnancy history: Gravida, Para, Abortions, Full-term, etc.
Patient's mother and father's age at the patient's birth
Patient's gestational age, place of delivery, delivery type, amniotic fluid, neonatal history (OFC, BW, BL, APGAR)
Is anyone using the Expanded History Form-2-CCC to capture this information?

We are not using the expanded cc form but our own custom forms. The forms do make use of obsterms to track the information however in order to bring forth the information, if needed, into subsequent visits.
