I was wondering what everyone is using for pediatric vital signs form? We just did an upgrade on Friday to the new templates (CCC forms) and it seems like the new forms are missing hearing, vision, U/A, peak flow. Actually, the adult vital sign form is also missing these fields plus multiple b/p check any suggestions?

For the Pediatric Vitals CCC I don't remember it ever have hearing, UA, Peak Flow. We have used separate forms for those items.
For the Adult Vitals click the Load Serial Assessment button. You will find the additional vitals on that form.

I would be happy to share the Vitals Signs form that we use for Peds. It does not contain areas to document hearing or U/A. E-mail me if you would like a copy [email protected].

Jessica, that is sweet of you thank you. Now I am starting to wonder how come we are the only office that is asked to put in hearing, vision, and U/A for well child exams ? This is a must especially when the office goes through an audit they look for this.
