I don't have the Problem-ccc form, is tis available in a clinical kit? sounds like it would be useful.

Do you have a C:\Program Files (x86)\Centricity Practice Solution\Client\CCC folder? We have CCC Basic + CCC 9.1 (the bonus version) so there's around 1200 files in that folder. If not, I believe you can load CCC Basic. I'm pretty sure CCC 9.1 is an extra cost item. I do not know if CCC Basic includes the Problems-CCC form. Perhaps someone else knows.
