Does anyone have a form that pulls in tests>date they were done with result>and order button if needs to be ordered? We are looking to address the following labs: HgA1c,LDL,HDL,Diabetic Eye Exam, TSH,Chlamydia Screening, PSA, Depressiojn Screen, and Mammogram & Pap.
The following alerts we would like if patients have the following dx:
CAD-Taking a Statin
CAD-with MI in past on Beta Blocker
CHF on beta blocker
CHF-on ACE
DM- HgA1c in 12 months
