I know how it sounds but hear me out. We refer out to a lot of physical therapy locations and a lot of times the patients don't know where they want to go or we have patients going out of state (back to school) and don't know the area. Either way we don't know what to put for the service provider. What are other groups out there doing to combat this?
We don't necessarily have the same issue as you describe but we limit who can update the service provider list so there are times when the service provider the patient is referred to isn't in the list.
To combat this we have a service provider called 'Other Service Provider'. For the service provider, the phone, fax, email, etc. are all left blank.
I think this would work in your situation as well.
We had this same issue come up. We actually created a service provider called 'Patient Preference Referral' and in the address we stated, 'Patient needs to notify clinic when provider is chosen'. Then we used our referral coordinators phone number and fax number. This has been working pretty good for us.

We created a Service Provider called "Insurance Contracted Provider".
