We have a phone note that had information in it and then was placed on hold. Later another user went in and removed much of the previous information, then signed the phone note. Is there a report or query that can be run to show what the information was prior to it being removed or what was removed from the phone note?
No
Nope unless you have a database backup between the two times. You can restore to a test sql server get in a gather what you need so it can be recreated. I've had to restore in the past because of situations like you explain. Besides disaster recovery its one of the reasons we run sql log backups every 15 minutes.
Like others have said above, I don't think there is much you can do for recovering or viewing that data. Just-in-case, below is a list of the items/options you can check to look for data/info that is otherwise hidden in a chart.
Chart Access Report
Used to see who has "touched" the patient's chart
- Open the Patient's Chart
- Click on Print
- Select Clinical Lists folder
- Select Chart Access Report
- Set your date range
- Click the binoculars icon, and select all users
- Click Preview to run/view the report
Show Contribution Text in Note
Used to see who made changes to a particular document in a patient's chart
- Open the Patient's Chart
- Find the Document you wish to investigate
- Right-click the document and select "Show Contribution Text in Note"
The text will display as Red Text in the document. When you're finished, right-click and de-select the "Show Contribution Text in Note", otherwise it will show for every document you look at until you turn it off.
Audit Reports
Other areas that may help, are the Audit Reports. These are located in the Chart Reports->Reports->MedicaLogic Folder->Audit Reports Folder
