I have searched through the arcives and can not locate an answer for this.
Has anyone developed a template for the SLUMS Examination? I have a Provider who put this on my desk last night and asked me to incorporate this in our system. I do not know if it is even possible or if I should create a handout instead? Any feedback what others have done?

Yes, there is one in our system attached to the Risk Factors form developed by Linda Noyes at ACHS. It is called SLUMS Mental Status Exam.
