Good morning, just wondering if anyone has a form for 'Screen for Child Anxiety Related Disorders (SCARED)' that they would be willing to share. I found obsterms so I was hoping someone might have one so I wouldn't have to "recreate the wheel."

On a related note, it would be nice to have a full PHQ-9 available. The form that's in there now has the PHQ-2 first. We have patients complete the full form, so we currently do them on paper and scan them in. We also use the SCARED form, so we would be interested in that as well if someone has one. =)

We are in the process of creating the SCARED form. Should have it ready in a couple of weeks. If you will email me your contact information i will share it with you.
