Can someone give me the best way to make a patients chart and registration sensitive where staff will have to give a reason why they are entering the patients chart.? Is this even possible?

You would need to give the staff the security "Access On Demand sensitive charts" instead of VIEW sensitive charts. With that setting, any time staff access's a patient that is marked as Sensitive they will be prompted to enter a reason.

** In Admin > System > Access On Demand:
- define your list of acceptable reasons to enter a sensitive chart
- check or uncheck whether you want users to be able to enter their own reason or stick to the list you define
- Add a note to staff on why they must enter a reason to enter a sensitive chart.
For example: The information you have requested is marked as sensitive. You must specify a reason for accessing this protected information. You many not access your own information or that of a dependent or family member.
**In Admin > System > User and Resource Management > Security:> Security by Group
(security by group is the easiest way to manage settings)
For the group:
- Chart > Access On Demand sensitive charts = should be checked
- Chart > VIEW sensitive charts = should be unchecked
- Registration > Access On Demand sensitive registrations = checked
- Registration > View sensitive registrations = unchecked
**Reporting:
- Chart Reports > Reports > MedicaLogic > Audit Reports > On demand access, by user
- put in a time frame, add users, and click preview

We limit the access to remove the sensitivity to Quality and Risk. Two people.
