Does anyone have a Sensory Motor Exam for Ophthalmology already created? It looks a lot like a grid that the doctor has drawn up for me. Thank you.

Christopher. Please let me know how my group can get the Sensory Motor Exam form. My doctor was impressed and said that she would like this form. Thank you.
