Sharing the attached VFE template for data entry of outreach activity. I have also included the Crystal Report I run to work with this.
The report keys on either the CDC or OTH C19 lab value being 'positive' or 'detected', or the c19 Prsum Pt having any entry. The idea is to ultimately have an entry in the "Patient Resolution' field indicating that the patient has returned to baseline/normal. But, the overall theme is to help us keep track of the positives and make sure someone is in touch regularly with the patient util they are better.

I figured I would share our process too. We built a form that looks a lot like what you did. We also are using the COVID-19 program ENLI has added to their Central Worklist product. So if a staff member screens a patient and the patient needs to quarantine due to exposure or suspected exposure, they are placed in the ENLI program. We then have staff who use the follow up form when they check in on the patient by phone until the end of the quarantine period.
