My users are finding this HTML form very confusing. If they have something prev recorded and they go to add in more history and click on "Insert selected value" - we find that the prev documentation is overwritten by the added info. Is this what other practices are seeing and if yes - is there a way to get around this? Thanks Peg
This is a known issue. I believe there is an SPR for it but I don't have the number at the moment. The only work around that I have found is to copy and paste the information that was typed into the comment box. You will need to do this for each visit if you want it to retain from visit to visit.

I don't see this behavior in EMR 9.10, but this only in test for us, so you have me worried.

I Open the social history form.
I have the following existing social history:
Patient currently smokes every day.
Patient has been counseled to quit.
Alcohol Use: Y
Regular Exercise: Y
Social History Reviewed: 10/24/2017
I click Yes for Drug use.
I then click insert current updates
I end up with the following in the little preview box, as well as in the document update (note the old history is present along with the new update of Drug use)
Patient currently smokes every day.
Patient has been counseled to quit.
Alcohol Use: Y
Drug Use: Y
Regular Exercise: Y
Social History Reviewed: 10/24/2017
We have not started using the form in question but we were thinking about starting to. There was a patch for CCC files that is only applicable to 9.10. Have you upgraded to that patch?
This is such a horrible work around too.
The other issue is that there is no quantification of alcohol intake on the HTML form, a functionally significant omission.

thanks for the additional feedback... with the new year coming we are in a rock and a hard place, upgrading and going to bugging forms or not being compliant. CMS????
