Lulu, can you try this version out? When you load it, there is a visit type that defaults to Primary Care. If you choose Consult, you will see spaces for a consult visit type and referring provider and PCP. I didn't understand your comment about time.

Hi,
I just installed the new TM version you sent me it looks good. Sorry on me not being clear enough regarding the additional minutes, so where it states visit details: it has start and end time and push time right below that it has Minutes____ Include time reviewing labs, recent hospital visits, and any other reports. When the doctor enters say the number of minutes after saving the document and signing it this appears of the Consult/OV note :
EXAMPLES: Visit Details
Start Time: 9:02 AM
End Time: 9:23 AM
I spent 3 minutes with the patient counseling and treating the condition(s) listed below.
Visit Details
Start Time: 9:34 AM
End Time: 9:48 AM

which the doctor says that is not correct : I spent 3 minutes with the patient counseling and treating the condition(s) listed below.
So what he did temporarily was not put anything on that Minute___ so that will not be coming up on the Consult/OV note. Can this be fixed? o

The note will show the start time and end time. The Push Time to Minutes button will push the difference from the start time and end time to the Minutes box, which is used for the phrase "I spent...". The box should be manually changed to reflect any additional time spent by the provider. So you could see a start time and end time that were 30 minutes apart, push the button to put 30 in the Minutes box, and then the provider could put 45 in the Minutes box, if they spent another 15 minutes with other tasks, so it would read "I spend 45...".
On your examples, is the provider not clicking the Push Time to Minutes button and then adding extra time? Or am I not understanding your comment?
Thank you for sharing this form, really quick question for some reason the check box for " Patient consented for audio or video type visit" once the form is signed the information is not transferred over to the note, is there something i am doing wrong ?
Maribel, I can't think of why that would not be in the note - you should see the obs term TELECNST populated and "Patient consent for audio or video type visit: Yes" in the note.
