Has anyone able to take a copy of the production database, apply it to a test environment and perform a upgrade without affecting the production? The issue i'm having is the license. I was told that if i use the license in the test environment, the production environment will error out. i'm not sure how accurate is that.

Speak with sales or your account rep. They should be able to supply you with a testing license. That is what we have done. Then when installing the centricity server use the new license for it. There is obviously some limitation, like only 3-5 people can log in, or somehting like that I do not recall exactly.
Joshua's suggestion is the "right" way to do it. However, for the record, I have done what you are suggesting and it worked fine. I ran a backup of my CPS 9.0.1 database and restored it to a new server. I then upgraded the new server to CPS 10.1.3 and told it to license over the internet. It licensed successfully and ran fine for more than 2 months while I did all my testing and training. The original 9.0 server also ran fine during that time, using the same license number.
