How are other clinics handling unsigned clinical list changes when sharing charts?

We ask our users to have the check box "Sign Clinical List Changes" checked when they put a document on hold. When we have a conflict with more than one unsigned document in the chart without the Clinical List Items signed, we contact the other user and ask them to sign the note or the clinical list changes at a minimum. If the other user cant be reached and the unsigned clinical list items are disrupting care, such as preventing a prescription from being entered, one of our support staff will sign the clinical list changes and leave the document on hold.
This process does have an issue when the unsigned clinical list document includes unsigned orders.
