Is there anyone who has built an Annual Wellness visit in VFE that they are willing to share or sell?

Here is ours annual medicare visit, maybe you can pull something off of it.
The original was in place before my time. I have added the adding of a order and populating obs terms to the respective visit type. Added a PHQ-9, and MMSE.

Adam, I tried to download from your link, and if you just click on it, it opens a new page (which is very weird) and downloads nothing. I right-clicked the link and did a "Save link as,.." and got a .dlg file with the correct title but no fields and no code. Can you verify your file posting? Thanks!

Here is a Medicare Annual Wellness Visit. It also includes English and Spanish Visual Signature Capture templates that feed data into the form. Also included is the VSC.LIB file for Visual Signature Capture functionality. The Visual Signature Capture integration is not required for the form to work. I'll try to get this written up and posted on the Marketplace soon.
