Does anyone have a VFE or HTML form/template; workflow; or process on how they are capturing these in their clinics? I have built a VFE form but we seem to have problems with the 10 year projection and the print out that the patients are given. We can do the GE CVS but that isn't what the providers want to give to their patients. We have a Mid-level that is dedicated to doing these in clinic but we have a bit that is falling through the cracks. Thank you in advance.

Thank you. I like the simplicity of this form. How did you come up with the handout? Did you involve the providers or did you go off of what CMS requires and present to the patients? I appreciate your help.
