Our providers would like this form linked to an observation term, specifically B12 and Testosterone Injections. Has anyone accomplished this yet?

There is no way to link obs terms to this form I found this out after adding over 200 meds and had a meeting with GE

This is a problem for us as well. You can link OBS terms to the form but you must have a separate set of OBS terms (medication, route, site, etc.) for each dosage of medication you have listed. You can request these from GE.
Another option is to create your own Med Admin form.
