Could the care team listed in patient portal be designed to pick up the patient's responsible provider rather than the last person to sign a chart document? Even, better, to meet PCHM, a comprehensive list of who is on the patient's team would be ideal, i.e. PCP, Nurse, CMA, etc..
As it stands, someone from medical records, IT or the scheduling team could be the last person to sign a chart document making the Care Team personnel listed completely incorrect. Thanks! Monica Orr HCH MD [email protected]
