Does anyone have the DLA-20 (Daily Living Activities) Adult Mental Health VFE form that they would be willing to share.

Hi Mari,

We are actively building out a Behavior Health Suite. If you will send me the paper copy of the form with the logic, we will build the form for you. We will do this at no cost to you. We will send you the source code so you can have your team edit it if you want. We should be able to deliver it in 7-10 days.
