Does anyone have some direction for clinicians on how to enter exceptions so that they are captured by CQR? We are now part of an ACO with measures we need to report (as well as MIPS reporting). Several of the measures have exceptions and are usually documented in the body of a clinical note, but not as discrete data. We need to be able to enter the exceptions appropriately so that the denominator reflects them accurately. Currently, all of our exceptions are "0".

Have you looked at the CQR manual to see if it gives specific instruction on what qualifies as exceptions for each measure? My guess is specific structured data needs to be present for a patient to qualify.
