I am curious which workflow people are using to hit the 15A piece of the transition of care measure? We are leaning toward sending via the patient portal but I haven't seen it update the MUactivitylog table (we are on the old version of Kryptic and were told there are issues related to this). We've found that printing is unmanageable due to the size of the documents and that save to file is just asking for "trashing" the document but I wanted to see what others are doing.
As for our general workflow we are moving forward with general referral orders entered in admin hold. We will then have a bucket user for every location and someone at each practice will be in charge of managing the flags that go to that bucket. We are instructing our practices to continue to do any and all activities related to referral ordering that they have done in the past (referral tracking, faxing documents to other providers etc) - this is additional workflow. As you all can imagine it has gone over well.

Because we are specialty practice we don't do many and are exempt but we are doing the workflwo to help 'prove' we are exempt. We are using the referral order and have created generic orders for 'Spine, Foot&Ankle, etc for the types we usually refer out for. The staff put in the phone note where the patient is going and generated records and images for the patient. The flag goes to referral desktop and we generate the workflow to count in CQR with save to file. We are adding the actual provider in service providers with a prefix of TOC- so they can easily find the practice or provider. Eventually some will have direct addresses and we can test that part too. Currently we are only receiving the electronic version and it is basically a CCDA and not super helpful for a referral. we need more information.
Thank you for your feedback, we are most likely following a similar workflow. I have my fingers crossed they allow a 3 month attestation period in 2015 and then most of our practices will be exempt. I will post our differences once we finish rolling out the workflow and hammer out the details.
