We are checking to see if anyone has any forms dealing with Alcohol Abuse/Risk Factors/Counseling that they would be willing to share. Thanks!

If you have CCC forms, you should have Risk Factors-CCC. That's what we are using.

We do and are currently using it, but it does not get detailed enough for some of our providers.

What version of CCC forms are you using? I believe in CCC v1.5 and up on the Risk Factor-CCC form the Alcohol and Drug questions are now separate tabs. They have Audit C and DAST screening questions. I think that is the detail your providers may be looking for.

Not the current version...We are upgrading everything before the end of the year. So will have to check this out once that occurs. Thank you!
