We have a provider who would like a report to track his allergy patients so that he can contact them if they have not been coming in for their injections. Most of our allergy patient documentation is done on paper so I am working on a form that would help to capture the information necessary to run a report.
Does anyone have a form/report in place that tracks patients to ensure they are coming in as needed for injections/treatments?
I have been brainstorming ideas and am not finding a way to run a report through Chart Reports-->Inquires. We don't have Crystal Reports to build a custom report.
These are the 3 things I need to factor in:
- Last Allergy Injection administration ("ALLERGY BY")--easy peasy
Heres where I run into trouble.......
- Consent signed ("ALLRGINJ1CON")
- Refusal/End Treatment that is after the last signed consent ("IMMUNSTOPDAT"). Patients could potentially refuse/end treatment and then later decide they want treatment. So I need a report that will look for a signed consent and also look to see that the patient has not refused/stopped treatment since that consent was signed
Thoughts or ideas on how I can make this work??
I would suggest you think about purchasing Crystal Reports; probably a $250-400 purchase. The Inquiry is ok, but no where near as flexible as Crystal. Do you know how to import a Crystal Report into your EMR System?

Actually, after thinking about the problem, I adjusted the report a bit, This one may work better.
Thank you so much for your responses! I tried to explain my logic but knew it was still going to be a little gray.
I think the second report (AL100a Allergy Patients) could work for me if it could be adjusted to show the date of the last value in each column rather than what was recorded for that OBS term.
I will also pass some information along to our IT director about purchasing Crystal Reports. I definitely feel it is something worth investing in.
