Is anyone else tackling the new CCM program from CMS? If so I would love to keep this thread going with what everyone is doing. I'm working on the documentation template now.

It mostly seems to be time sheet documentation. Wondering how you are going to keep a chart update opewn for weeks at a time while different staff, providers document in it? I can see how the care plan could be done and kept in the chart, as is a more one time thing. The consent would have to be paper, signed and scanned in , unless you had electronic patient signature technology. Could Phreesia tablets do that?
The AAFP has developed some nice patient consent, and Care Plan forms if you haven't seen them. Family Practice Management will have an article in the Jan/Feb 2015 issue, which has been pre-released electronically if any of your physicians are AAFP members.

We are moving forward with this in 2015. I would be interested in hearing what others are doing to document track the time spent each month.

Has anyone tackled building a template in Centricity for tracking time?

I created a template back when we were working for PCMH. This template would work with a little tweaking for a care plan. As for tracking time you simple need a start time and stop time and a window for the care coordinator to transcribe what was done in the time. I am working on a form now and would love to collaborate with anyone else also.

Just a little note you can print a consent from from the system and send it to word and use your tablet or topaz signature to capture the patients signtaure. The topaz has free software to do this. Then simply import it back in to your system. #NOMOREADDONS #NOMOREFEES

kaplan32 - Just wondering if you would be willing to share what you have so far for your form for tracking Chronic care management. We looked at the kryptiq solution however the provider who is interested in doing this doesn't really want to spend the money if we don't have to and with everything else we have had going on here, I haven't had a chance to really work on a form for this.

We use a form from a company called Polaris Danforth Corp. http://polarisdanforth.com/
It seems to work well for our provider.

We are also implementing a program for a Chronic Care Coordinator. Wondering how other are documenting this in the EMR? Custom form or using a GE form to capture this data.

We created a custom form I can show the screen shots that we use. We created a form for the providers care plan and then a form for the nurse care plan

Kaplan are you willing to share this form? We are going live with our Care Coordinator program 8/1 and we do not have a template to use yet, and yours look beautiful.

YES, JUST SEND BE YOUR EMAIL. YOU WILL HAVE TO MODIFY THE OBS TO WHAT YOU ARE TRACKING IN YOUR OFFICE
