Hello,
We are interested in connecting with a group that has successfully implemented CCM. We would like to hear about other's experience with the items listed below. In turn, I'd be share any workflow challenges we have tackled and custom content that is of interest to the group.
1.) Implementation experience.
2.) How did you scale?
3.) How the CCM care team is structured?
4.) Panel sizes?
5.) Have you developed any “graduation” criteria?
6.) How do you consider success? Or what are your population goals, or KPIs if any?

bthurman- below are some answers to your questions.
Hello, We are interested in connecting with a group that has successfully implemented CCM. We would like to hear about other's experience with the items listed below. In turn, I'd be share any workflow challenges we have tackled and custom content that is of interest to the group.
1.) Implementation experience.- We are still in the implementation process, we are trying to come up with community services for patients if needed, get the billing process up and running, and define our patient population as we do not have Lightbeam up and running yet.
2.) How did you scale?- not sure what you mean by this.
3.) How the CCM care team is structured?-we have 1 full time care coordinator and one part time. We are going to assess the patient of one provider and go from there to determine if more Care members are needed. Once we are done with a visit note we then sign it and route it to the PCP for review and signature.
4.) Panel sizes? - unsure of what you are referring to...
5.) Have you developed any “graduation” criteria? 6.) How do you consider success? Or what are your population goals, or KPIs if any? -We plan on following up with patients on a monthly basis by phone or face to face visit. At that time we will revisit the goals and see if improvement has been made. I think the graduation criteria will be on a case by case basis depending on compliance to goals.

How is everyone tracking time? Leaving the document open all month? Excel sheet? If excel, would you mind sharing? We are just getting started, and I am trying to avoid over complicating this.
