Hello,
We are interested in connecting with a group that has successfully implemented CCM. We would like to hear about other's experience with the items listed below. In turn, I'd be happy share any workflow challenges we have tackled and custom content that is of interest to the group.
1.) Implementation experience?
2.) How did you scale?
3.) How the CCM care team is structured?
4.) Panel sizes?
5.) Have you developed any “graduation” criteria?
6.) How do you consider success? Or what are your population goals, or KPIs if any?
