We are starting to explore the need and implementation of care coordination. We are currently a mental health and substance abuse provider. We were wondering if any other organization has already started using cps12 to accomplish care coordination. If so, would you be willing to have a conference call with us so that we could hear about your experience with this? Thanks.

You should look into a product by Visualutions that will allow your care coordination needs to be addressed.
