I was wondering if anyone knows or can recommend a product that interfaces with Centricity that allows patients to complete electronic forms (such as past medical history/surgical history) through the use of a IPad as well as electronic signatures. Thank you

Phreesia.com
We are just in the implementation stages of doing this. We have our patients currently using the Phreesia Pads for Demographics and about to start phase 2 of using it to collect clinical data (medical history, ROS, social history, etc).
It does not use iPads it uses Android based tablets with fall proof protection enclosure (like old Leap Frog tablets for kids).
They can sign documents electronically using the Phreesia pads.
Patient can also fill out paperwork from home on their pc or smart phones.
It is not a perfect solution, but we are working through the issues and see lots of potential. There are some bugs with how it posts payments that is our current challenge to come up with work arounds.

Patients can also view and sign documents such as HIPAA, Financial Responsibility, etc. and those will be placed in the appropriate section of the Patient Record.

I can also recommend Visual Signature Capture. The device requirements are flexible and if you have some basic experience developing forms making the documents is simple.
