I have one provider who uses a custom form for his dictation. He has asked that I add an additional edit field for each of the following: PMH, PSH, FH and SH. He has been meeting the character limit and still has additional info to add. All of these boxes are currently attached to OBS terms with default previous. I have used the same OBS terms that the CCC forms use so that it will pull forward for our other providers who use the CCC forms.
I thought about setting up the second box to a separate OBS term but then the full history would not populate for the other providers who use the CCC forms.
Then I thought about setting the boxes up to document variables and having multiple boxes push to a single OBS term. But again I worry it won't populate the full history for other providers who use the CCC forms.
Any one have any suggestions?

Yeah we have hit the Text Limit with OBS terms before as well. For all the History forms (FH SH PMH PSH), we have had to insist that providers use the checkboxes provided, so that the obs terms are uniform and have good data, and any additional comments can be dictated into a separate obs term. This way, other providers get the data they need, and the provider in question can add long comments all day long.
Good luck, it can be hard to explain why click boxes are important to a provider who loves dictation. I'd also try to figure out what makes his notes so long, and try to find a solution so he can still get all his info in. but sometimes, we just have to have our medical director put his foot down for certain workflows.
