We are a multi-specialty clinic of about 35 providers. We have our own laboratory with an interface between CPS and Orchard. Here is our dilema.....we currently have approximately 15,000 lab orders that are dated for after 10/1/15. These orders have ICD-9 codes attached that will need to be converted somehow to the correct ICD-10 code prior to 10/1/15. Does anyone have a similar system that you have a plan in place or a solution on how to manage making the changes prior to 10/1/15?

We have Orchard for in-house lab as well. Orchard is asking $5000 for ICD-10 conversion.
Did anyone installed ICD-10 conversion package from Orchard?

From a technical standpoint, I assume the orders are being exported via Linklogic/DTS? If I were looking into this (which perhaps I will be), I'd try creating a cross reference file.
I'd probably take a list of all our orders over the last year and base the cross reference file on that. As multi-specialty, that might be a big list, but it doesn't seem insurmountable.
As you change your order codes to ICD-10, they will simply bypass the cross-reference.
Depending on the timing of transition of each side, you could also have the exact opposite cross reference. You can be transitioning the order codes to ICD-10, and have the cross-ref convert to ICD-9 for you. Then when the LIS is ready for ICD-10, simply reverse the columns so you are converting ICD-9 to ICD-10 for orders.
That's my initial idea. No idea if it's viable without testing. If someone does this, let me know the outcome.
CPS has the option in Linklogic to either send ICD-9 only or Both. So that is just a click, but the process in Orchard maybe another issue.

Are you referring to check box to "Export diagnosis with only ICD 9 codes" under Change Orders Constaints options. Currently I have as checked. So if you uncheck it will send both ICD 9 and ICD 10 codes in HL7 order Export?

Yes, that is correct. If you uncheck it, both ICD 9 and 10 codes will be exported in the HL7. The ICD 10 will be first followed by the 9 code. It will be in the DG segment. If there is not a 10 coded associated then the 9 code will be in the first section.

No problem, I have tested and my HL7 is fine going to orchard with 10 code but I have some work to do in Orchard to get it to accept the diagnosis. I had similar problem sometime back, had to transfer codes from icd-9 to icd-10. On top of that I wanted smooth transition from 9 to ICD-10. This company helped me big time. You should follow the link to talk to them.
