Is anyone using the PRAPARE form provided by NACHC? I'm just trying to familiarize the facility with a functioning workflow. Aside from using the form (we're actually going to put ours on tablets) I'd like to hear/see what others are using for the Health Worker documentation.

We have several clients using Visual Signature Capture (VSC) and the PRAPARE as well as other Social Determinants of Health (SDOH) formats. All bring the OBS in to Centricity.
