I was wondering if anyone has a good system for collecting birth outcome information for total and CDS reporting?

We as well are working on the Prenatal measures and are working on the tracking of birth outcomes. At this time we have a prenatal coordinator gathering the birth outcome data manually. I am not sure what systems you have, we have Visualutions and CPS. We just had a training on the Case Management tool in registration from Visualutions which has an area in this template that gathers the birth information, allowing the data to be captured in our UDS reports. We were utilizing i2iTracks, entering the data manually and then providing the numbers at the end of the year, this new way cuts a step and is hopefully more accurate. I would love to chat, we feel like we are continuously having to customize templates in order to meet measures.

We also have this problem. We actually have a report we call 'missing babies'. It sounds terrible, but is really just those pregnancies missing outcomes.
We originally tried using the case management section in Visualutions, but since it requires extra steps because it does not pull from the chart. We more less stopped using it when we did not have grant money to pay for staff to track the information. Although, I think we have started using it again because we tried to have forms that could capture the information but pregnancy info is so challenging in obsterms.
I really wish that athenahealth would develop a better way to document pregnancy data instead of having to figure out complex coding in forms to have when one pregnancy ends and another one starts. (okay I will get off my soap box now).
To follow-up on the original question - we are thinking of using Docutraks to add obs terms on birth weight to the mother's chart when the delivery discharge note comes through. I'm pretty sure the barrier will be the relatively low percentage of discharge summaries we receive. Other thoughts or comments?
