HPI form Provider Notes Missing

Is is a known issue that information entered within the HPI form randomly disappears from the patient chart? I thought it could be related to the unsigned clinical list defect but I don't see anything on the repaired or non-repaired report that should suggest the HPI form has an issue. I've been told that the form is known to be "clunky" and I'm struggling with that.
