Just wondering if anyone could share what their workflow was for putting medications on hold. For instance if a providers asks the patient to stop the medication for a week or so while on an antibiotic what do your providers do?
Unfortunately, not everyone does the same. If it's short term as you described we don't do anything other than document in the phone note or office note the instructions and reasoning for the hold. If the patient thinks they're having a reaction to a medication and we want them to hold it for a week, but we'll be following up with the patient to see if resolve of their symptoms, we would leave the medication on the med list and write ON HOLD next to the medication name. If after follow-up the medication is going to be discontinued we would remove it from the med list, stating the reason why. If the patient is to continue on the medication, we would just remove the ON HOLD descriptor after it.
