Want to create a 'registry' of a couple hundred patients who will be receiving care management services. For many reasons, it seems a best approach would be to use an ICD-10 code, even if we make it custom - assign a slightly different text to associate with it.
Thus, been looking at the Z- codes, but can't seem to find a good one. Had been thinking about Z51.5 = encounter for palliative care. However, upon further thought, this does not seem to be a good match.
So, is anyone assigning these patients a Problem Code? Or, are you simply utilizing an OBS Code?

We use obsterms to track to our care management program patients. We have a couple different care management pools and they each have their own obsterm with Enrolled/Not Enrolled as the available values.
We also have associated obsterms to each pool to track care, referrals, etc.

I would recommend that you use an OBS Term.
