If patients move, leave your practice but still see specialists within your office, or are discharged completely, how do you track and report this in PM? We are looking for examples and ideas to track and document and distinguish the reason why the patient left, without using the Active, Obsolete, Inactive patient statuses because the patient search list defaults to active so then we increase the risk of duplicate patients, etc.
Do you use a proxy provider? How do you get your reporting? How does staff know whether or not they can be scheduled? What do you do?
Reasons (for example):
Transferred, Care
Changed, PCP
Moved, out of area
Discharged, PCP
Discharged, HHMA
